The Continuum Heart Institute offers a wide range of approaches for optimal cardiac health. Our cardiac health services are geared toward: anyone at risk for cardiac disease and diagnosed with cardiomyopathy, heart failure or stable angina; those who have had a heart attack, bypass surgery, angioplasty/stent or valve repair/replacement.
Our cardiac health programs give individuals the hope for fuller and more satisfying lives. The staff teaches participants how to manage their disability and helps them to develop self-confidence and an overall sense of well-being. An individually tailored regimen will combine medically supervised aerobic exercises with education and support, enabling patients to live more comfortably, become more active and regain their independence.
Each candidate is evaluated by a physician and is required to have a cardiopulmonary stress test within six months prior to enrollment or following any cardiac procedure. This test defines an individual’s current functional capacity and will help determine the exercise regimen. In addition, each patient is assessed by a physical therapist and an exercise physiologist so that the appropriate level of exercise can be prescribed. These assessments take into account all medical limitations, so as to develop a beneficial and safe exercise regimen. We individualize the plan to meet each patient’s personal needs and goals.
EXERCISE SESSIONS. These sessions are geared toward helping individuals strengthen their muscles and improve the function of their heart and lungs. Patients are guided through each exercise session from start to finish. Ongoing communication between members of the rehabilitation team and the patient ensures that functional capacity improves, and that the patient is satisfied with the program and his or her progress.
Exercise regimens incorporate individual factors, such as medical history, medication requirements and lifestyle. Individual exercise programs are prescribed to increase upper and lower body strength, as well as overall endurance and stamina. Our therapists teach patients how to safely increase their activity levels, emphasizing exercises that condition the respiratory, cardiovascular and musculoskeletal systems.

Patients exercise in fully equipped, state-of-the-art gyms that include treadmills, stationary bicycles, upper body ergometers, stair climbers, free weights and weight machines. NUTRITION COUNSELING. A well-balanced diet is key to maintaining a healthy heart. Sound nutritional habits can help control weight, reduce cholesterol levels and decrease blood pressure. Our nutritionists present lectures for participants on a variety of relevant topics such as weight management, cholesterol and triglyceride reduction, low-fat meal planning and the latest nutritional research findings. They are available to answer questions about developing and maintaining a healthy diet and also can be contacted for private consultations. STRESS MANAGEMENT. Chronic stress can be a major factor in the development of cardiac disease. In our stress management classes, participants learn effective techniques for reducing stress and overcoming tension. These classes combine gentle yoga stretches, breathing exercises, deep relaxation and imagery. Participants work with certified practitioners at their own pace and according to their needs. EDUCATIONAL LECTURES. The Heart Institute offers a comprehensive schedule of education seminars for participants. Health team members provide valuable information on many topics, including understanding coronary artery disease, medications, breathing techniques, stress management and relaxation training, nutrition, personal and family relationships, and travel. In addition, participants can take advantage of our health education library, comprised of numerous books, articles and videos on all aspects of coronary artery disease and rehabilitation. SUPPORT GROUPS. These sessions offer participants an opportunity to develop relationships with others who share their concerns. During our support group meetings, participants discuss emotional and psychological issues associated with coronary artery disease, explore the day-to-day reality of living with a severe chronic illness and share practical information. We offer these cardiac health services in both Manhattan—at the Center for Cardiac and Pulmonary Health—and in Brooklyn—at the Center for Cardiac Health.
